according to Walkscore.com the most walkable U.S. cities are:

1. San Francisco

2. New York

3. Boston

From personally experience, I would say that New York is much more walkable than San Francisco. SF has a lot to offer to walkers but the destinations are spread out more than New York. NY has many more grocers, drug stores and other common needs stores that city dwellers would desire. SF also has those killer hills and to get from the famous Piers over to the Embarcadero you need a serious pair of hiking boots. My thighs and calves are aching just thinking about my time spent walking in SF.

Boston is kind of laughable that it is on the list since it feels like it takes a week to walk from one end of the city to the next. But this is also coming from a person whose spent the majority of her time in Boston sitting on the T, the commuter rail, or driving in. Most walking in Boston for me is spent walking from the Red Line over to Bank of America Pavillion (ugh!) or from the Back Bay all the way to the North End (or up, down and all around Cambridge).

For international cities I would nominated London and Dublin, but I do need to get out a bit more into foreign territory.
